Graft Vaginoplasty
The graft technique, a common method for sex change surgery, involves using the patient’s own genital skin to create a new vagina. It offers several advantages, including avoiding abdominal surgery and minimizing scarring. The depth of the vagina varies depending on the patient’s natural anatomy, with an average depth ranging from 5.5 to 6 inches. This technique is considered the standard approach for initial sex-change surgeries due to its simplicity and low risk. However, proper care is essential post-surgery, particularly during the first year, which involves regular dilation of the new vagina.
The graft technique is employed to construct a comprehensive external female genitalia, encompassing the clitoris, clitoral hood and frenulum, labia majora, labia minora, and urethral orifice. This technique achieves a functional and adequately deep vagina without the need for abdominal surgery. Additionally, it effectively preserves the sensory nerves located at various significant locations.
When it comes to graft vaginoplasty, there are certain concerns to be taken into account. One issue is that the vagina may become constricted or shallower if it is not properly dilated during the procedure. Additionally, there may be a lack of lubrication inside the vagina. Another drawback to be aware of is the potential for additional scars if grafting from other areas is necessary. For patients with limited genital skin, alternative methods using skin from the groin or thigh may be utilized, but this can result in extra scars in those areas.
With the grafting technique, patients can expect a hospital stay to last approximately 9 days. During the first three days after surgery, the patient will remain in bed and consume soft meals. The wound will be covered with two layers of gauze, and a urinary catheter and drainage will be utilized. On the fourth day, the outer layers of gauze will be removed, and the patient will be permitted to leave the bed. On the seventh day, the remaining gauze, urinary catheter, and drainage will be removed, and the patient will receive instructions for vaginal dilation. If no complications arise, the patient will be discharged from the hospital. Following surgery, it is recommended that the patient rest for one month and perform vaginal dilation on a daily basis to aid in recovery.
The final outcome of the procedure may not be apparent until six months later. Full healing requires time. The swelling and discoloration will gradually decrease. The vagina will naturally take shape after the surgery and conform to your body’s unique characteristics. Factors such as the color of the scrotum and the amount of fatty tissue will influence the appearance of the genitals. It is important to note that the end result varies for each individual.
If you are dissatisfied with the appearance or function of your genitals after the surgery, it is advisable to consult with your plastic surgeon or urologist. They can determine if further surgery could result in improvements. Generally, corrections are not performed within 6 months of the initial surgery. It is recommended to wait at least one year.
